Quizzes offer an engaging method to test your respondent's knowledge or for generating more leads, offering shareable content to attract people to your brand.

With Formly, you can build and personalize your own quiz in minutes, calculate scores and automatically send quiz results to your respondents.

What’s a score quiz?

With a Score quiz, you can show respondent different endings by assigning points to their responses and a score range to each ending. As respondent go through your form, their scores will be added up, and they’ll see an ending based on their score range when they submit the form.

Score quizzes are great for segmenting leads by scoring them or for assessing knowledge.

How to create a quiz

Follow the steps below to create a quiz you can send to your respondents.

1. Create the first Question by clicking the + Add question button and selecting Multiple choice question type. Enter the question text and type in four possible answers.

2. Now to upload the image. In the Question tab of the right-hand sidebar, click the + button next to Image or video.

3. Upload your image, then choose among the layout options to position your image in the question.

4. Click the + Add question button to add another question and repeat the above process to add all your questions and images.

5. Now let’s set up the score calculations. To do this, go to the Logic tab and select Calculator. Select the correct answer – Limited Time Offer: 20% Off in this case – from the drop-down list and add 1 to the Score. Repeat this process for each of the questions.

6. Let’s set up some custom Ending screens to show the Respondent's their scores at the end. With this in mind, we’re going to create three Ending screens:

0-1 Correct Answers: Novice Networker - You’re just getting started, lots to learn!

2-3 Correct Answers: Marketing Maven - You’ve got some skills, keep honing them!

4-5 Correct Answers: Lead Legend - You’re a marketing maestro, keep up the great work!

The end result will look like this in the Create panel.

7. To create an Ending screen, click + next to Endings on the bottom left side panel and enter the text. Type @ then select score from the drop-down to display your Respondent’s score in the Ending screen.

Add another Thank you page if you want to display a different results page based on the number of points the respondent receives.

8. Now go to the Display Logic tab and set visibility logic to show respective Thank you screen based on the score range and click Save.

In this example, we show the respondent the first 'Thank you' screen if the score is between 0-1 and the second 'Thank you' screen if the score is between 2-3 and the third 'Thank you' screen if the score between 4-5.

Note! Make sure that your score ranges don’t overlap with each other. For example, you cannot create a score range from 0 to 1, and then the next one from 1 to 5. The correct score range would be 0 to 1 and then to 5. You can, however, create a score range for a single score, for example, 5 to 5 if all answers are correct, and 0 to 0 if all answers are wrong.

9. Let’s make our quiz a bit prettier. Add a Welcome Screen, Short text (for respondent name) by clicking + Add question.

10. Email results to respondents and yourself (optional)

Email notifications are only available on paid plans (Silver/Gold/Enterprise plans).

You can send email to yourself, team members or respondents and even make them conditional based on quiz scores.

10.a If you would like to send an email to respondent then you’ll need to add an email address question in the quiz by clicking the + Add question button and selecting Email question type.

10.b Create a personalized email

In your form Settings tab, go to Email notifications

10.c You can select Self email notifications, Respondent email notifications or Conditional email notifications. Learn more about email notifications.

In this example, we will select Conditional email notifications as we want to send different email text based on the score range.

Click on Set conditions and the popup will show up where you can specify the required conditions.

Click on 3 dots to add condition. As you can see, Formly’s logic feature is built into custom email notification, so you can use add condition to set up new logical rules, just like you would when adding logic to your form.

10.d You can now fully edit the email body text and can also recall responses in Notification email message body by typing "@" and choosing score, name etc from the recall information menu.

10.e Include PDF

Tick "Include PDF" if you want to receive completed form as a PDF email attachment.

10.d Click Save to set this custom email notification.

That’s it, you have successfully setup custom email notifications.

As a result, the personalized email received by each respondent will show their name, how many points they scored in the quiz.
