You build your form out of questions. Here are all the different question types that you can use to create your form.
You’ll find all the questions in the Create panel. To get here, log in to your account, open an existing form, or start a new one, and the Create panel will open.
To add a question, click the + button on the left to see all the different question types:
You can also change an individual question's type from the drop-down menu under Question settings, in the right-hand sidebar.
Inputs
Short Text
If you want a short written answer from respondent such as a name, address etc, this is what you need. You can also choose a min and max character limit for answers.
Long Text
This is ideal if you want people to write long answers. You can also choose a min and max character limit for answers, like with Short text questions.
Date & Time
The Date question allows Respondents to pick a date in this format: 30 Dec, 1989
Respondents can pick a time using a 12-hour or 24-hour format, such as 01:30 PM
or 13:30
depending on which hour format you have enabled in the question settings.
You can also switch off time toggle in the question settings if you want respondents to only pick a date and vice-versa.
Number
This only accepts numbers as an answer. The number question supports positive whole numbers, decimal points and negative numbers.
You can also specify a min and max number limit for answers.
Contact
This only accepts correctly formatted email addresses as answers. If respondents tries to add text in the wrong format, they'll see a warning message, and have to try again.
Tip! Enable the "Verify Email" toggle if you want respondents to complete a 2-step email verification process before they can submit the form.
Phone number
The phone number question lets you collect contact details in a snap. You can choose a default country code that will be prefilled to save time for your respondents.
The respondent can also type or select a country code from the dropdown menu, and each country is configured so they can only enter a correctly formatted number.
The phone numbers will be available in the Results
tab in similar format: +1(212)456-7890
depending on the country code.
Website
Use this to collect websites and links from your respondents.
Choices
Multiple Choice
When a question requires respondents to choose from a list of options, you need a Multiple Choice question.
In question settings, you can make your question required, allow multiple selection, randomize (so the order of your choices will change every time your form is opened), add an "Other" option, and change your choices alignment.
Tip!
Clicking the "Other" option opens a text field where respondents can type in their answer.
If you have a long list of answer choices, you can also add them in bulk by simply copying and pasting them from a spreadsheet, for example.
Dropdown
Use the dropdown question type to offer a long list of answer options, from which your respondents can choose one.
In question settings, you can set the question as required, randomize or alphabetize the list options, or add an image or video.
If you choose to activate Multiple selection, you can then choose to allow respondent unlimited selections, an exact number, or give them a range ("Choose 2-5 of the following", for example).
Image Choice
The Image Choice question is a visual version of Multiple Choice, you can add images to each option with a Image Choice question.
In question settings, you can make your question as Required, turn the picture labels on and off (turn off if you want no text under your images). You can set your images to Supersize, Magnify image, Multiple selection and Randomize choices.

Two Choices
Use this for closed questions – where there are only two choices - Yes/No, Happy/Sad, Like/Dislike and Male/Female.
Rating & Ranking
Slider
A Slider is a graphical input field that allows respondents to select any number that falls between the slider’s range. You can define your slider’s range in the Min and Max values.
The Default value input field allows you to choose what number the slider will be set at by default.
The Step input field allows you to set up the size of the intervals users can choose from on the main slider. If you choose the Step size to be 10, the slider will allow users to select number at intervals of 10.
The Prefix and Postfix input field allows you to write something before and after the slider's value.
Rating
A rating question is a simple and visual way of rating things. You can use stars, hearts, trophy and many more!
You can choose the number of options (between 1-10) in your rating scale, and select the symbols you want to use.
Note! For Smileys symbol, the number of options are fixed to five in the rating scale.
Opinion scale
This lets respondents give a rating on a numbered scale from zero to ten. You can define the range with the steps tool. The default is 10, which gives you a scale of 1-10. The smallest number of steps you can have is 2, giving you a scale of 0-1.
Optionally, you can also add labels to the left, middle and right of your scale.
Matrix
This allows respondents to input data in a grid-like format. It can be used to create a Likert scale or for questions that involve multiple statements or criteria with a common set of response options.
You can have input type as - Single select, Multi select, Text entry and Dropdown.
Note! You can add Logic to Matrix questions. But you can't Recall information from this question type.
Media
Signature
Use this to capture simple electronic signatures for contracts, offers, payments and more.
Signatures are saved securely as image files on your Results screen. Click on the signatures files to open and download them. In Google sheet and CSV (Excel) export, signatures will appear as a link, which can be downloaded.
File upload
Include a File Upload question in your form to let respondents attach files to their answers.
The File Upload question supports all file types, with a maximum file size of 10MB.
Respondents can also upload files from their mobile device. If they’re viewing the form on a phone or tablet, they can either upload a photo from their library or take a picture with the camera and upload it.
You can then download the file via the Results panel.
Note!
If you’d like to allow people to upload multiple files to your form, then add separate File Upload questions for each one.
If the respondent needs to upload a file that’s larger than 10MB to your form, you can ask them to upload the file to a repository service—like Dropbox or Google Drive and let them paste the link into a Website question. This will allow you to collect the links and download the files later.
If you want respondents to download a file from a form, simply upload your file to a repository service such as Google Drive or Dropbox, then copy the link to this file, and paste it into your form (in a Statement question, for example).

Form structure
Welcome Screen
Start your form on the right note with a Welcome Screen. Captivate your audience with a nice image, explain the purpose of your form, introduce yourself, and more. Make a memorable first impression!
Statement
These are useful when you want to say something without requiring a response. You can add a quote or an inspiring message to your form with a Statement. These are ideal for creating a pause or transitioning to the next section of your form.
Thank You Screen (Endings)
A Thank You Screen is the final screen your respondents see after submitting a form. You can show different Thank You screen based on the answers selected, price/scores achieved by the respondent. Learn more about Thank You Screen (Endings)
Question settings
Every question type has a settings menu that you'll find on the right side of the screen in the Question panel.
Here you can set your question to Required, set the maximum number of characters for responses, add an image or video, and more depending on the question type.
